This application is designed for a 3-Week Agriculture Summer School at Dine College Land Grant Office Extension and Outreach. 

 

Documents Needed

  1. Admission Application
  2. Land Grant Program Application
  3. Official Certificate of Indian Blood (CIB) or Tribal Enrollment Card, if enrolled in a Federally Recognized Tribe 
    • Must include a stamped seal.
    • Only official, sealed copies are accepted (no photocopies or scanned copies).
    • Submit by mail or hand delivery.
  4. Official In-Progress High School Transcript
    • Must include the GPA and official seal/stamp.
    • Transcripts must be sealed and unopened when mailed or hand-delivered.
    • Electronic transcripts (e-transcripts) are accepted if digitally certified and sent directly by your school via Parchment Transcript Exchange.
  5. High School Concurrent Permission Form
    • The form must be signed by the student, parent, and school officials.
    • The completed form must be emailed to admissions@dinecollege.edu
  6. FERPA Disclosure
    • A FERPA Disclosure Form is a document that gives permission to share a student’s educational records with specific individuals or organizations.
    • It is based on the Family Educational Rights and Privacy Act (FERPA), which protects the privacy of student records.
    • The form identifies who is allowed to access the student’s information (e.g., parents, guardians, or third parties).
    • It specifies what information can be shared (such as grades, enrollment status, or financial records).
    • The student must sign the form to authorize the disclosure.
    • Schools and colleges use this form to ensure they are legally compliant before releasing any protected information.
