- Official Certificate of Indian Blood (CIB) or Tribal Enrollment Card, if enrolled in a Federally Recognized Tribe
- Must include a stamped seal.
- Only official, sealed copies are accepted (no photocopies or scanned copies).
- Submit by mail or hand delivery.
- Official In-Progress High School Transcript
- Must include the GPA and official seal/stamp.
- Transcripts must be sealed and unopened when mailed or hand-delivered.
- Electronic transcripts (e-transcripts) are accepted if digitally certified and sent directly by your school via Parchment Transcript Exchange.
- High School Concurrent Permission Form
- The form must be signed by the student, parent, and school officials.
- The completed form must be emailed to admissions@dinecollege.edu
